Kiyotaka Itani was a Japanese anime director working for
Studio Cockpit who was born in 1965. He is possibly most well known for directing Pokemon's infamous seizure flash episode,
Cyber Soldier Porygon.
On a company vacation to Akiyama-gou in 2010, it was discovered that Itani's blood pressure was over 200. After the trip, Studio Cockpit's president Hidenori Yamaguchi hurriedly arranged a medical examination but Itani skipped it because he was busy with his work. The following year, in September of 2011, Itani's co-workers at Studio Cockpit lost contact with him. Shortly thereafter, his apartment was investigated by his landlord, a police officer, and
Kimitake Nishio of Studio Cockpit where he was found dead. His funeral was arranged by Yamaguchi and held in Tokyo.
It is theorized by some that Itani's untimely death was one of the factors which lead to Studio Cockpit's dissolution in 2016.
